Sioux Valley Dakota Nation will be providing $150 for COVID-19 assistance for our off-reserve membership. This will help with the purchase of PPE and COVID-19 related expenses. This payment will be in the form of a gift card and will be mailed to you.
The deadline for application is November 10th, 2022.
Applications will be done online at the link (https://www.surveymonkey.com/r/SVDN_Nov_Assistance) in the post above, or you can scan the QR Code to be directed to the application.
The requirements are one gift card per household and you must be 18+.
You will need to provide the following information:
– Full Name (first AND last)
– Status Number (10 digits)
– Address (including FULL postal code)
***Note: Community Mailbox Address will NOT be accepted***
– Telephone Number
– Email
– Date of Birth
– Other SVDN Members that reside in the household
