– Camera is available or you can provide your own photo

Child(ren) Registration
– application form
– Long Form Birth Certificate with Both Parents Names
– copy of both parent’s identification
– Guarantors Declaration Form

SCIS (white card) Application
– application form
– Guarantors Declaration form
– copy of photo I.D.
– SCIS PhotoAPP (Send Photo & Signature)

Band Transfers
– SVDN Band Transfer Form
– ISC Transfer Form
– Copy of photo I.D.
– Signed & Dated letter written by individual
