Sioux Valley Dakota Nation would like to assist the Oyate with some back to school clothing for your child(ren).
Please complete this form if you have SVDN registered children from 4 years old to 17 years old in your care that are registered for school.
*By completing this form you consent to sharing the information collected to determine your eligibility for this provision.
Eligibility Criteria:

- Child must be a registered member of SVDN

- Child must be registered in school for the 2025-2026 school year (proof/confirmation is required)

- Children must not have received any other back to school assistance from any other organization

- Child must be 4 years old or 17 years old as of September 1st, 2025
Conditions Apply:

- Proof of School Registration must be submitted prior to pick up

- Receipts must be submitted (email scanned copy/picture to: info@svdngovernance.com or dropped off in person)

- Only one verified parent/guardian can sign for pick up, NO pickups on behalf of others

- Deadline for registration is September 15, 2025 at 4pm, no new applications will be accepted after this time.
Distribution will occur after the registration deadline, your patience and understanding is appreciated during this process
